Established patients may also use this form for any non-urgent questions. For the fastest communication, please log into the MindWeal app and send a secure message to your care team.

This form is for non-emergency inquiries and general questions. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency room (ER).

Office Hours and Response Time
Our office operates Monday to Friday, 8 AM – 5 PM. We are closed on weekends, national holidays, and select regional holidays. Messages received during business hours are typically addressed within one business day, but it may take up to two business days for a response.

New Patients
To schedule a new patient visit, click HERE. Once your account is created, the platform will guide you through each step, including signing consents & completing your medical and psychiatry history. You can also self-schedule your visit directly through the app.
