Do you charge for completing any paperwork or forms that I might need filled?


If your child is required to take medication in school and the school mandates a form to be completed by their provider to facilitate this, there will be no charges for completing these medication administration forms. Additionally, if we compose a letter as part of your child’s treatment plan during the day of their clinical visit, there will be no separate fee beyond the charge for the visit itself. Examples of such letters include advocating for the patient’s needs in school (for instance, requesting accommodations for ADHD, executive functioning deficits, or autism spectrum disorder), seeking additional testing for learning disabilities, or writing a letter to the child’s pediatrician to coordinate care.
However, there will be charges for any paperwork not recommended as part of the treatment plan by your provider or not completed on the day of the visit. To learn more about these charges, please click HERE.
